Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection where employees were exposed to fall hazards of between approximately 8 and 20 feet, a ladder not being extended at least 3 feet above the landing and ground crews not using hard hats when working in areas below other employees about 8 feet or so above them.
