29 CFR 1926.20(b)(2): The employer did not ensure that a system of frequent and regular inspections of the job sites, materials, and equipment were made by a competent person: a) Job site, 5003 Lenox Oval Drive, Pittsburgh, PA: On or about November 14, 2023, during gutter installation activities, the employer did not ensure that a system was in place to identify and correct safety issues. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection where employees were exposed to fall hazards of between approximately 8 and 20 feet, a ladder not being extended at least 3 feet above the landing and ground crews not using hard hats when working in areas below other employees about 8 feet or so above them.
