29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.  a) On or about November 14, 2023 the employer did not utilize documented energy control procedure for the Seyi SAG-330 330 ton press during troubleshooting in that employees utilized an undocumented method of isolating the energy by removing an electronic key that was attached to the die block placed between the dies.

29 CFR  1910.178(l)(3):Training program content. Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace.  (l)(3)(i)(G) Fork and attachment adaptation, operation, and use limitations; (l)(3)(ii) Workplace-related topics: (l)(3)(ii)(C) Load manipulation, stacking, and unstacking; (l)(3)(ii)(I) Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation.  a) On or about November 2, 2023, the employer did not adequately train powered industrial truck operators in topics to include but not limited to (l)(3)(i)(G), (l)(3)(ii), (l)(3)(ii)(C), (l)(3)(ii)(I) on the safe operation and handling of the Galbreath self-dumping hopper, in that employees were not trained on the proper procedures of how to manipulate the hopper if it does not self dump. On or about November 2, 2023 employees were exposed to a  caught in hazard from the hopper tipping while trying to force the hopper to tip by getting off the forklift and using a hammer to strike the hopper.

29 CFR  1910.217(b)(7)(iii):A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) shall be supplied with the clutch/brake control to select type of operation of the press. Fixing of selection shall be by means capable of supervision by the employer.  a) On or about November 14, 2023, the employer did not control the fixing of selection between off, inch, single stroke, and continuous for the Seyi SAG-330 330 ton part revolution press in that the key was left in the press for operators to control exposing employees to pinch points hazards.

29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) On or about November 14, 2023 the employer did not conduct an annual periodic inspections of the Seyi SAG-330 330 ton part revolution press energy control procedure to ensure that employees were following  the machine specific procedure during servicing and maintenance.    The Fram Group Operations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1539147 , citation number 2, item number 1 and was affirmed as a final order on December 28, 2021, with respect to a workplace located at 851 Jackson Street Greenville, OH 45331.  The Fram Group Operations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1500948, citation number 1, item number 1 and was affirmed as a final order on February 22, 2021, with respect to a workplace located at 851 Jackson Street Greenville, OH 45331.

29 CFR  1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about November 14, 2023, the employer did not ensure employees were adequately trained in the methods and means necessary for energy isolation and control while troubleshooting the Seyi SAG-330 330 ton part revolution press in that employees were trained to isolate the press by removing the motor key and installing the die block which relies on interlocks and control circuitry for servicing and maintenance of the press.  The Fram Group Operations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1539147, citation number 1, item number 3 and was affirmed as a final order on December 28, 2021, with respect to a workplace located at 851 Jackson Street Greenville, OH 45331.

The employer shall insure by adequate supervision that correct operating procedures are being followed.  a) On or about November 14, 2023, the employer did not adequately train, instruct, and supervise employees in the safe methods of work in that employees were not trained on the established energy isolation procedures, and were trained to isolate the Seyi SAG-330 330 ton part revolution press using the die block.  The Fram Group Operations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.217(f)(2), which was contained in OSHA inspection number 1539147, citation number 1, item number 5 and was affirmed as a final order on December 28, 2021, with respect to a workplace located at 851 Jackson Street Greenville, OH 45331.
