29 CFR 1926.451(d)(5)(iii):  Suspension scaffold support devices such as cornice hooks, roof hooks, roof irons, parapet clamps, or similar devices shall be secured against movement by tiebacks installed at right angles to the face of the building or structure, or opposing angle tiebacks shall be installed and secured to a structurally sound point of anchorage on the building or structure.  Sound points of anchorage include structural members, but do not include standpipes, vents, other piping systems, or electrical conduit:  a) On November 14, 2023, cornice hooks supporting a two point suspension scaffold were not tied back to a structurally sound point of anchorage on the building.  No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion.  Safe points of anchorage unclude structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights:  a)  On November 14, 2023, vertical lifelines were not connected to a fixed safe point of anchorage; lifelines were connected to the tie backs of the scaffold.  No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(3)(iv):  Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system:  a)  On November 14, 2023 vertical lifelines were attached to ropes being used as suspension scaffold tiebacks. 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) On November 14, 2023, employees were exposed to falls hazards while working around unprotected skylights.   Galewood Tuckpointing & Roofing LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(4)(i) which was contained in OSHA inspection number 1562117, citation number 1, item number 2, and was affirmed as a final order on January 6, 2022, with respect to a workplace located at 5005 Enfield Ave, Skokie, IL 60077  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
