Sound points of anchorage include structural members, but do not include standpipes, vents, other piping systems, or electrical conduit: a) On November 14, 2023, cornice hooks supporting a two point suspension scaffold were not tied back to a structurally sound point of anchorage on the building. 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On November 14, 2023, employees were exposed to falls hazards while working around unprotected skylights. Galewood Tuckpointing & Roofing LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(4)(i) which was contained in OSHA inspection number 1562117, citation number 1, item number 2, and was affirmed as a final order on January 6, 2022, with respect to a workplace located at 5005 Enfield Ave, Skokie, IL 60077 In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
