29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step. On or about November 8, 2023 and times prior thereto, employees were exposed to fall hazards while working from the top step of a four (4) foot fiberglass "A" framed folding step ladder near the edge of a second story residential new construction home exposing them to a fall hazard of approximately 14 feet to the ground below.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. On or about November 8, 2023, employees were exposed to fall hazards and were not provided a training program to include ladder safety training on how to recognize and eliminate ladders hazards.
