29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. On or about November 15, 2023, with regard to an establishment located at N1070 Quality Drive, Greenville, WI 54942, the employer did not ensure that an employee operating the Mitsubishi LP Forklift had received re-training every three years as required by the standard.
