29 CFR 1926.651(c)(2): A stairway, ladder, ramp, or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) On or about October 19, 2023, as the controlling employer, Campbell Construction, LLC did not ensure that subcontractor employees were provided with a safe means of egress while working in the trench excavation that was approximately 10 feet or more in depth. Subcontractor employees were using a sloped section of the excavation for egress that was not protected, exposing employees to cave in hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) On or about October 19, 2023, as the controlling employer, Campbell Construction LLC, did not ensure that subcontractor employees were protected from cave-in hazards while working in a trench without adequate protective systems in place. Subcontractor employees were working in a trench hat had 10 foot near-vertical walls, signs of sloughing, and material and equipment on the edge of the trench. This condition exposed subcontractor employees to cave-in hazards.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a) On or about October 19, 2023, as the controlling employer, Campbell Construction LLC, did not ensure that subcontractor employees were protected from falling materials or equipment while working inside the trench as the spoil pile was directly at the edge. Subcontractor employees were directly below the edge of the trench excavation where the excavator was operating on top of the spoil pile, exposing employees to struck by hazards.
