Euclid Avenue, Elmhurst, Illinois, the employer did not certify that forklift operator(s) had been trained and evaluated as required by paragraph (l) with certification records. An employee was operating a Traverse Lift F-644 forklift to deliver lumber to the second level. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were exposed to struck-by hazards from lumber and the forks of a forklift while conducting framing activities.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. (a): On or about November 15, 2023 at the jobsite located at 621 S. Euclid Avenue, Elmhurst, Illinois, employees were exposed to fall hazards while performing framing activities and the employer did not certify that employees had been trained to recognize fall hazards and the procedure to be followed in order to minimize those hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

ABD Construction Pros, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.102(a)(1)), which was contained in OSHA inspection number 1619405, citation number 1, item number 1, and was affirmed as a final order on November 2, 2022, with respect to a workplace located at 672 Euclid Ave., Elmhurst, IL 60126.
