29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) At #18411 Hwy 144 E, Richmond Hill, Georgia: On November 15, 2023, the employer exposed employees to fall hazards, in that fall protection was not used while employees performed roofing work with an eaves to ground fall distance of 10'8".

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:   a) At #18411 Hwy 144 E, Richmond Hill, Georgia: On November 15, 2023, the employer exposed employee to fall hazards, in that training in fall hazards and the procedures to be followed to minimize such hazards was not provided to each employee that performed framing activities.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:  a) At #18411 Hwy 144 E, Richmond Hill, Georgia: On November 15, 2023, the employer exposed employees to fall hazards, in that two extension ladders were used to access the framing and roof, that did not extend at least three feet above the upper landing surface.
