Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.  a) On or about 11/10/2023 in the docking area of the shipping department, the employer did not ensure that the aisles utilized by the forklifts to load and unload trucks, maintained a sufficient safe clearance in that pedestrians were exposed to struck-by hazards due to chemical drums being stacked three high in walkways causing a blind spot where powered industrial vehicles were operated.  CSHO last observed the hazard on 11/16/2023 (day of the opening), boxes continued to be stacked creating a blind spot.
