29 CFR 1910.157(c)(4): Portable fire extinguishers were not kept in their designated places when not being used.  On or about November 16, 2023, and at times prior thereto, two locations with-in the warehouse that were marked as having fire extinguishers did not have fire extinguishers in place, exposing employees to hazards such as but not limited to smoke inhalation and burns.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse.  On or about November 16, 2023, and at times prior thereto, multiple pallets were hanging off of storage racks, pallets were stacked crooked, and in some areas, product falling over and off the pallets, exposing employees to struck by hazards.

On or about November 16, 2023, and at times prior thereto, an employee operated the onsite forklifts Yale 50VX and Komatsu 25 forklift without being trained and evaluated through a certification program, exposing employees to struck by hazards.
