On or about November 16, 2023, and at times prior thereto, two locations with-in the warehouse that were marked as having fire extinguishers did not have fire extinguishers in place, exposing employees to hazards such as but not limited to smoke inhalation and burns.

On or about November 16, 2023, and at times prior thereto, multiple pallets were hanging off of storage racks, pallets were stacked crooked, and in some areas, product falling over and off the pallets, exposing employees to struck by hazards.
