29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)   (a) On or about November 16, 2023, the employer did not ensure the competent person was conducting frequent and regular inspections of the job site, to ensure Personal Protective Equipment such as hard hats were being worn,  fall protection was being used,  that ladders were installed properly, and job site conditions were maintained safe.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about November 16, 2023,  the employer did not ensure that the employee assisting in roofing operations (ground workers) were protected from potential falling objects from above by the use of a protective helmet. This exposed the employee to a struck-by hazard from a height of 12 foot, nine (9) inches or more by building material remnants and tools located above the ground level employees.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about November 16, 2023, employees conducting roofing work on an 8:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately  12 feet, 9 inches on the North side of the roof, and 25 feet on the West and East sides of the roof near the ridge onto grass and concrete.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about November 16, 2023, the Werner Extension  ladders  erected on the North side of the structure, at the garage, and along the front side of the house while being used for access to the 8:12 pitched roof, did not extend at least three (3) feet above the  eave, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately twelve 12 feet, 9 inches onto grass and concrete.
