29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about November 15, 2023, employees were working in a excavation and the siderails of the ladder being used to egress from the excavation did not extend at least three (3) feet above the landing.  This exposed the employees to a fall hazard.

29 CFR  1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted.  On or about November 15, 2023, employees were working in an excavation where there was more than two feet of earth material below the bottom of the shield system being used.  This exposed the employees working in the excavation to a cave-in hazard.  The H&H Plumbing and Utilities, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(e)(2)(i), which was contained in OSHA inspection number 1645205 , citation number 2 , item number 1 and was affirmed as a final order on June 15,  2023, with respect to a workplace located at 14137 Broadway Ext Oklahoma City, Oklahoma 73034.
