29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: T&J Concrete Foundations, Inc. - 25943 County Highway O, Cadott, WI. On or about the beginning of January 2024, the employer did not report an amputation that occurred in calendar year 2023 within 24-hours of being notified that the injury was classified as an amputation. (a) On or about September 14, 2023, an employee suffered an amputation of the right ring finger tip above the first knuckle, Case log number 4 . In approximately the beginning of January 2024, the employer was notified that the injury was classified as a partial amputation of the fingertip and failed to report the injury to OSHA.
