29 CFR 1926.760(a)(1): Each employee engaged in steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On or about November 15, 2023, and at times prior thereto, employees were exposed to a fall hazard while laying down bridging at a height above 15-feet without the use of fall protection.

On or about November 15, 2023, and at times prior thereto, employees were exposed to the hazard of falling over 15-feet while performing steel erection work without being provided with connector training.
