29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. On or about November 17, 2023, at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold, fourteen feet (14) six (6) inches above the ground, was exposed to fall hazards due to lack of a guardrail system.

On or about November 17, 2023, at the above addressed jobsite, an employee using pneumatic nail guns was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.  Joel De Jesus Ocampo dba AAA Roofing was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1598288, citation number 1, item number 1 and was affirmed as a final order on July 19, 2022, with respect to a workplace located at 2260 High View Rd., New Lenox, IL 60451.

Joel De Jesus dba AAA Roofing was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1598288, citation number 1, item number 2, and was affirmed as a final order on July 19, 2022, with respect to a workplace located at 2260 High View Rd., New Lenox, IL 60451.
