29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water.:  (a) 15660 North Columbia Blvd., Portland, Oregon 97203, Terminal Berth area, Vessel Global Round, Port Midships Deck, Willamette River, on or about November 17, 2023 and at times prior thereto: Employee were exposed to fall hazard to vessel deck from unguarded edge from gangway vessel entry. Which potentially result in broken bones, sprains, and abrasions to head, body, and extremities.

(a) 15660 North Columbia Blvd., Portland, Oregon 97203, Maintenance Facility, on or about November 17, 2023 and at times prior thereto: Employees were exposed to a TCM, Propane, Model FG 35 787, serial # A35 L99448, power industrial truck with no manufacturers operations manual, fork controls missing function indicators, inoperable brake lights, turn signals, and exposed electrical components. Which would potentially lead to loss of control of vehicle striking employees or other objects. Resulting in electrical shock, Struck by, pinch points, crush by, vehicle accidents, and various equipment malfunctions with injuries and disabilities.
