Inc, 306 Wolf Street, Syracuse, NY 13208, on or about 11/20/2023, Employees were operating a propane powered Nissan 50, Model #CPJ02A25PV that was unsafe due to the quantity of Carbon Monoxide, CO, it was emitting inside the facility. Ambient air concentrations of CO ranged from approximately 42 ppm to 87 ppm during and after the operation of the propane powered Nissan 50 forklift.

Stone Shop, Roma Tile Co. Inc, 306 Wolf Street, Syracuse, NY 13208, on or about 1/9/2024, Polisher was working in the stone shop wet polishing silica containing stone and the employer did not conduct an exposure assessment. Polisher was exposed to 40.56 micrograms per cubic meter (mcg/m3) of silica, eight-hour time weighted average (TWA), which is above the OSHA action level of 25 mcg/m3. The sampling period was for 468 minutes. A zero increment was included for the 12 minutes not sampled. b.) Stone Shop, Roma Tile Co. Inc, 306 Wolf Street, Syracuse, NY 13208, on or about 1/9/2024, Cutter was working in the stone shop wet cutting silica containing stone and the employer did not conduct an exposure assessment. Cutter was exposed to 32.53 micrograms per cubic meter (mcg/m3) of silica, eight-hour time weighted average (TWA), which is above the OSHA action level of 25 mcg/m3. The sampling period was for 450 minutes. A zero increment was included for the 30 minutes not sampled.
