29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) St Henry Tile Co, Inc. St. Henry, Ohio: On or about October 25, 2023, the employer did not ensure the RodChomper Inc. model #BRE8 bender was guarded in such a manner as to prevent employees' hands from accessing pinch points created by the rotating equipment and rebar rods being formed. Employees performing bending tasks in this condition were exposed to pinch point hazards.

Washington St. St. Henry OH 45883: On or about October 25, 2023, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2023. The descriptions in Column C (job title) Column D (date), and Column F lacked the required information, including, but not limited to the object or task that caused the injury and the part of the body affected.
