29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and de-energization of the machine or equipment have been accomplished:  (a) Corrugated Department - On or about November 11, 2023, the employer did not ensure that the Post-4 machine was de-energized prior to performing set-up for the next production run. Employee(s) were exposed to un-expected start up of the machine resulting in exposure to mechanical hazards during set up of glue heads, carrier wheels, and guide rails.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a) Corrugated Department - On or about November 11, 2023,  the documented procedure for the Post-4 machine, in the employer's lockout/tagout program, did not adequately outline the scope, purpose, and authorization to be utilized for the prevention of unexpected energizing, start-up or release of stored energy. Employee(s) were exposed to un-expected start up of the machine resulting in exposure to mechanical hazards during set up of glue heads, carrier wheels, and guide rails.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:  (a) Corrugated Department - On or about November 11, 2023, the employer did not ensure that the Post-4 machine was guarded to protect against laceration and amputation injuries. The ingoing nip points created by the belts, pulleys, and carriers did not have any guarding to prevent employees from accessing the zone of danger.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.219(c)(2)(i):All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  (a) Corrugated Department - On or about November 11th, 2023, the employer did not ensure that a rotating shaft at ground level was guarded. Employees were exposed to caught-in hazards from the rotating shaft at the terminus of the Post-4 machine.  This item was corrected during inspection.
