29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Altium Packaging - 405 Nestle Way, Breinigsville, PA - On or about November 17, 2023, Dura-lite Blow Mold Machine Operators working blow mold machines on lines 1 and 2 that produce milk jugs, were exposed to 8 hour time weighted averages in excess of 85 dBA. The employer did not complete annual audiograms within 365 days of their last baseline or annual audiogram. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use: a) Altium Packaging - 405 Nestle Way, Breinigsville, PA - On or about November 17, 2023, employees utilize XL-55, a corrosive cleaner. The employer had an eyewash station that was blocked by various objects that prevented employees from immediate emergency use for quick drenching of the eyes. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
