29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: At a worksite located at 12 East 1st Street, Parkville, MO, 64152, employees were exposed to fall hazards. The employer failed to instruct the employees in the recognition and avoidance of hazards created by open holes in the floor after protective coverings were removed from the holes. The hazard was located on the lower level of the structures roof. This hazard was most recently documented on or about 11/16/2023.

29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. At a worksite located at 12 East 1st Street, Parkville, MO, 64152, employees were exposed to fall hazards. The employer exposed employees to these hazards by allowing them to work near exposed holes in the roof of a structure under construction without fall protection or mitigation measures in place. This hazard was most recently documented on 11/16/2023.
