A) On or about October 26, 2023, procedures contained in "Lockout/Tagout-Energy Control Program" did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the requirements of 29 CFR 1910.147(c)(4)(ii)(A)-(D).  Employees were exposed to caught-in hazards of moving elements when performing servicing/maintenance tasks such as adjusting the photo eye of Bread Pattern Former #3.  B) On or about March 14, 2024, procedures contained in "Lockout/Tagout-Energy Control Program" did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the requirements of 29 CFR 1910.147(c)(4)(ii)(A)-(D). 29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A) On or about October 26, 2023, the hazardous electrical and pneumatic energies were not isolated on the Bread Pattern Former #3, and lockout devices were not affixed to each isolating device prior to an employee being exposed to caught-in hazards of moving parts when performing servicing/maintenance task such as adjusting the photo eye.  B) On or about March 10, 2024, the hazardous electrical energy was not isolated on the Bun Slicer section of the Model 90 Autoload Machines on Bun Lines 2, and lockout devices were not affixed to the isolating device prior to employees being exposed to machine hazards when performing maintenance tasks such as removing and installing circular bun slicing blades during set-up or change-over.  C) On or about March 14, 2024, the hazardous pneumatic energy was not adequately isolated on the Bun Line 2 Bagger Machine because a lockout device was not placed on the pneumatic energy isolating device prior to an employee being exposed to machine hazards when performing maintenance on an air cylinder.   The Pan-O-Gold Baking Co. was previously cited for a violation of the equivalent Occupational Safety and Health Standards 29 CFR  1910.147(d)(2) and 29 CFR 1910.147(d)(3) which were contained in OSHA inspection number 1414346, citation number 1, item number 2a and citation number 1, item number 2b, and were affirmed as a final order on 11-22-2019, with respect to a workplace located at 1000 Wilburn Rd., Sun Prairie, WI 53590.
