29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees a. On or about November 20, 2023, at the jobsite, employees were exposed to crush-by hazards, in that a suitable means of access and egress was not provided to employees working approximately 50 feet away from the ladder positioned on the North end of the excavation. The excavation was approximately 6 ft - 2 inches deep and 66 feet in length, with near vertical walls, in Class C soil.
