On or about: November 20, 2023 Location: 5 Port Imperial Boulevard Weehawken NJ 07086 a) Scaffold platform in duct shaft was not erected by qualified person. Scaffold did not have guardrail system or a secure and leveled work platform. Employees were exposed to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about: November 20, 2023 Location: 5 Port Imperial Boulevard Weehawken NJ 07086 a) Employee who was performing drywall installation in duct shaft on scaffold platform fell approximate 38-feet onto duct work and pipe below. Employee was not protected from falling off scaffold platform by use of a personal fall arrest system or guardrails.
