Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe lacerations, amputations, and struck-by hazards associated with the cut-off machinery and/or other objects:  a) On or about November 20, 2023, an employee was exposed to hazards associated with coming in contact with a rotating cutting blade, being struck-by thrown objects, and/or reactive forces, including kickback, while operating a Stihl TS 420 K12 cut-off machine that was equipped with a carbide-tip circular toothed saw blade.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    a) On or about November 20, 2023, the employer did not instruct or provide instruction to employees in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or minimize the hazards associated with operating a cut-off machine.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
