AMPP Incorporated, worksite located at 28271 Cedar Park Boulevard, Perrysburg, Ohio 43551: On or about November 14, 2023, the employer did not ensure employees were protected from entering the point of operation while bending small parts on the RG-2 Amada 35 ton hydraulic press brake, Model # 3512LD. Without an effective form of guarding, employees performing these tasks were exposed to amputation hazards. b. AMPP Incorporated, worksite located at 28271 Cedar Park Boulevard, Perrysburg, Ohio 43551: On or about November 17, 2023, In the press area, the employer failed to adequately the guard point of operation, ingoing nip points, and robotic movements on the Amada Astro-100 Model # FBDIII-8025, ME- 2 Automatic Press Brake. Employees operating on and/or around the the Press brake and robotic accessories in this condition were exposed to struck-by and caught in hazards. c. AMPP Incorporated, worksite located at 28271 Cedar Park Boulevard, Perrysburg, Ohio 43551: On or about November 17, 2023, In the press area, the employer failed to adequately the guard point of operation, ingoing nip points, and robotic movements on the Amada Astro-100 Model # 1253M, MH- 1 Automatic Press Brake. Employees operating on and/or around the Press brake and robotic accessories in this condition were exposed to struck-by and caught in hazards.

The employer did not provide and maintain sufficient access which could hinder the ready and safe operation and maintenance of equipment.
