In the unisex bathroom of the main office building located at 5863 West Beaver Street in Jacksonville Florida: On or about November 21, 2023, and at times prior the employer exposed employees to electrical shock hazards, in that the faceplate of the 20 ampere lightswitch outlet was missing. b. In the hallway of the same main office building, specifically the light switch that controls the VP and HR's office's lights: On or about November 21, 2023, and at times prior the employer exposed employees to electrical shock hazards, in that the faceplate of the 20 ampere lightswitch outlet was missing. c. In the reception area of the same main office building: On or about November 21, 2023, and at times prior the employer exposed employees to electrical shock hazards, in that the faceplate of the 20 ampere lightswitch outlet was missing.
