29 CFR 1910.303(b)(7)(i):Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment: On or about November 17, 2024 An employee engaged in maintenance activities at 1000 West Bond Street, Lincoln, Nebraska was exposed to electrocution and electrical shock hazards in that the buss bar within the electrical panel was not covered to prevent contact.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation: The employer failed to maintain injury and illness logs . This violation was most recently documented at a worksite located at or near 1000 West Bond Street, Lincoln, Nebraska on or about November 17, 2023. The employer did not ensure that the OSHA Form 300 for the calendar year of 2023 was completed as required, in that; Block F of the OSHA 300 form was not completed with sufficient detail. Block F requires the employer to describe the injury or illness, and object/substance that directly injured or made person ill (e.g. Second degree burns to the arm/hand from coming in contact with a buss bar within an electrical panel).
