29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 535 Irving Avenue, Port Chester - NY: Employees were exposed to fall hazards of approximately 26 1/2-feet while using an extension ladder to access a scaffold and a roof. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about, November 27th, 2023.
