29 CFR 1926.100(a):At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats. On or about November 20, 2023, three employees were passing cut wood, without wearing a hard hats, above their heads to coworkers that were working on the roof; exposing the employees to struck-by hazards.

29 CFR 1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. On or about November 20, 2023, three employees were using a ladder to climb to the roof in which the side rails of the ladder did not extend three feet above the walking surface of the roof, exposing the employees to fall hazards.
