29 CFR 1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected: On or about November 27, 2023, the employer did not ensure that welding generator cables were in good condition. The cables had numerous splices where the cable insulation was replaced with electrical tape. This condition exposed employees to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Construction Site: On or about November 27, 2023, employees were exposed to electric shock hazards in the following instances: a) A Bobcat 250 Welder Generator #1 (Serial # NA020169R); the connect torch cable to electrode terminal and work weld output terminal had broken strain reliefs and outer jacket damage. b) A Bobcat 250 Welder Generator #2 (Serial #MF350255R); the connect torch cable to electrode terminal had a broken strain relief and outer jacket damage.
