29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about October 19, 2023 and at times prior thereto, 2 employees were exposed to fall hazards while working on aerial lift without lanyards and harnesses at a height of approximately 35 feet.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On or about October 19, 2023, and at times prior to, an employee worked on a commercial building roof with no fall protection. This exposed the employee to fall hazards
