29 CFR 1926.451(c)(1)(iii): Ties, guys, braces, or outriggers were not installed to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, such as a cantilevered work platform, was applied or was transmitted to the scaffold.  Location: 288 Mile Hill Road Tolland, CT. On or about November 27, 2023 a scaffold was not equipped with outriggers, ties, braces, or guys to prevent tipping where the following eccentric loads were applied:   a) Pulley used for raising material from ground to work area located on the Northeast corner of the scaffold. b) Cantilevered work platform and walkway from the scaffold to the work area located on the South side of the scaffold.  Safeside Chimney and Duct Cleaning was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1511572, citation number 1, item number 1-a and was affirmed as a final order on August 3, 2021, with respect to a workplace located at 203 Cedar Ridge Drive Glastonbury, CT 06033.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:  Location: 288 Mile Hill Road Tolland, CT. On or about November 27, 2023 a scaffold base plate was placed directly on the ground without a mud sill or other adequate firm foundation. In addition, the same leg was also equipped with a pulley apparently used for lifting material from the ground to the work area, exposing employees to a fall hazard from scaffold collapse due to an unstable foundation.  Safeside Chimney and Duct Cleaning was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1511572, citation number 1, item number 1-b and was affirmed as a final order on August 3, 2021, with respect to a workplace located at 203 Cedar Ridge Drive Glastonbury, CT 06033.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 288 Mile Hill Road Tolland, CT. On or about November 27, 2023 employees were exposed to the following hazards:  a) An employee fell from a height of approximately 13 feet to ground level from a 12-12 pitch roof. The employee was standing on a "pitch hopper" but was not otherwise protected by guardrail systems, safety nets, a personal fall arrest system, or alternative fall protection measures under 29 CFR 1926.501(b). b) An elevated walkway approximately 16 feet above ground level used to access the work area from scaffolding was not protected by guardrails, safety nets, or alternative fall protection measures.  Safeside Chimney and Duct Cleaning was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1511572, citation number 1, item number 2 and was affirmed as a final order on August 3, 2021, with respect to a workplace located at 203 Cedar Ridge Drive Glastonbury, CT 06033.
