29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to injuries from inadequately maintained and labeled storage rack systems:  Location: Center warehouse Northeast side of room  a) On or about November 28, 2023, and times prior to, the employer did not ensure that all rack columns were anchored to the floor with anchor bolts adequate to resist all applicable forces.

29 CFR  1910.28(b)(1)(i)  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Storage mezzanine:  On or about November 28, 2023 Employees were exposed to fall hazards due to missing guardrails on the storage mezzanine when they were picking and or putting away material that was stored there.
