29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Decorative Panels International at the address of 2900 Hill Avenue, Toledo, Ohio: On or about November 9, 2023, the employer did not ensure that machine/equipment specific energy control procedures were developed and utilized to control potentially hazardous energy sources before an employee performed repair tasks on the Wrap Line Stacker. The employee, attempting to reapply a displaced drive belt without the benefit of energy control was exposed to crushing hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a. Decorative Panels International at the address of 2900 Hill Avenue, Toledo, Ohio 43607: On or about November 9, 2023, the employer did not ensure that employees tasked with service and maintenance activities were adequately trained in the application of energy control in which isolate and control hazardous energy sources during both maintenance and servicing tasks on the Wrap Line Stacker. Employees performing these tasks without the benefit of adequate training were exposed to crushing hazards.
