29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): In the alternative 29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: (a) Lower shop area, near mezzanine stairs; On and before November 29, 2023, the employer did not ensure that employees were protected from accidentally contacting energized 120-volt AC conductor wires that protruded from an existing electrical box that was located near a walkway used during construction activities.

On or about November 29, 2023, the employer did not provide the required information in column F to identify what object/substance directly injured each employee and did not place a mark in column G, H, I, or J to classify the case as a death, days away from work, job transfer/restriction, or other recordable case on OSHA Form 300 for calendar year 2023. (a) On or about November 22, 2023, while working at the Ladysmith School, an employee received a hernia. (b) On or about October 3, 2023, while working at the ECI shop, an employee received a head/scalp abrasion, and the case type was not classified. (c) On or about September 27, 2023, while working at the ECI parking lot, an employee received a right knee laceration, and the case type was not classified.
