Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a.)On or about November 29, 2023, the employer did not ensure that sufficient safe clearance was provided for mechanical handling equipment such as the Toyota Rider Pallet Electric Lift Truck; Model 8HBE30 loading and unloading double stack containers, gaylord over 6' and the APCs' and the pedestrian employees including expediters working in areas such as, but not limited to; dock door #11 to prevent the pedestrian employees from being exposed to struck by hazards.

As a substitute for the fixed wiring of a structure. On or about November 29, 2023, the employer did not ensure flexible cords were not used as fixed wiring, located at panel 208 and column M5, an outlet strip with a computer, display monitors, and other pieces of equipment were plugged into the outlet strip to supply power.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a.) On or about November 29, 2023 the employer did not ensure that emergency exits in the docking area were free of obstructions. Two exits were partially obstructed by carts.
