29 CFR 1910.263(e)(1)(viii): Horizontal dough mixer(s) were not equipped with a full enclosure over the bowl while the agitator was in motion: a) Mixing Department: Employees were exposed to an amputation hazard from the unguarded mixing blades while operating three Globe mixers that were not equipped with bowl guard wire cages, on or about 11/29/2023. b) Icing Department: An employee was exposed to an amputation hazard from the unguarded agitator mixing blades while operating Hobart and Globe Mixers that were not equipped with wire cage guards, on or about 11/29/2023.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Mixing Department. The employer did not visually inspect the Globe and Hobart mixers electrical cords that were missing their ground pins, on or about 11/29/2023.
