Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or burn hazards:  Or in the alternative:  29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:   a) On or about November 26, 2023, and at times prior at the Lundberg Federal Wellsite at or near Gap Road and Fifth Street Northwest at or near Killdeer, North Dakota, employees removing end caps from casing tubes and drifting the pipe, employees were exposed to fire/burn hazards where vapor producing flammable solvents were used to clean pipe threads, and sources of ignition were present.  Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to:  1. Follow the American Petroleum Institute Recommended Practice 54, Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations section 5.2 "Fire Safety". 2. Follow the American Petroleum Institute Recommended Practice 99, Flash Fire Risk Assessment for the Upstream Oil and Gas Industry section 4 - which requires that a Fire Risk Assessment be conducted to assess the risk of flammable vapors reaching an ignition source. 3. Ensure that LEL or 4 gas monitors are provided and used in all activities where flammable or combustible liquids and/or vapors may be present.
