29 CFR 1926.453(b)(2)(v): A harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. a) Fuller Home Building Inc. DBA Fuller Building Group, 215 Arch St, Meadville, PA 16335- On or about 11/30/2023 and at times prior thereto, employees were removing slate tiles off the side of a large multistory building from a JLG 600S (Serial #10830036) aerial lift. Employees did not attach to the anchorage point on the aerial lift while working approximately 40-feet above the ground. Fall protection harnesses and lanyards were observed on two employees in the lift basket without being connected to an anchor point.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. a) Fuller Home Building Inc. DBA Fuller Building Group, 215 Arch St, Meadville, PA 16335- On or about 11/30/2023 and at times prior thereto, employees were removing slate tiles off the side of a large multistory building from a JLG 600S (Serial #10830036) aerial lift. Employees were observed operating a JLG 600S (Serial #10830036) aerial lift without being trained by a competent person to recognize hazards associated with the work. Employees were exposed to fall hazards of approximately 40-feet above ground level.
