29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph: a. 631 Grove Street, Jersey City, NJ 07310. The employer failed to ensure all employees assigned to operate powered industrial trucks were competent to do so through the successful completion of training and /or other forms of evaluation. An employee suffered two broken fingers as a result of jumping out of a Type E, Raymond Model 420-03577 forklift after losing control of the equipment. Violation occurred on or about November 30, 2023
