29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  At 315 E Ash Street, Perry, Florida, on or about November 30, 2023, the employer exposed employees to eye injuries by flying debris, in that employees using pneumatic nail guns to nail roof shingles were not wearing safety glasses.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a. At 315 E Ash Street, Perry, Florida, on or about November 30, 2023, the employer exposed employees to a 13 feet fall hazard, in that no conventional fall protection system was being utilized while 14 employees performed roofing activities on a 6:12 pitched roof.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  At 315 E Ash Street, Perry, Florida, on or about November 30, 2023, the employer exposed employees to a 13 feet fall hazard, in that employees doing roofing work on a 6:12 pitch roof were using a Multi-Position ladder that did not extend 3-ft above the landing surface.
