PSS Manufacturing Company, Inc., 405 School Road, Denver, PA - Welding Section - Employees conducting machine press operations adjacent to the welding section were not protected from welding arc flash, on or about November 30, 2023.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.1200(e)(1): The employer did not develop, implement and maintain at the workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and had not developed or implemented a written hazard communication program which included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  a.) 29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a.) PSS Manufacturing Company, Inc., 405 School Road, Denver, PA - On or about March 25, 2024, the employer did not institute a continuing and effective hearing conservation program when employees' nose exposure exceeded an 8 hour time weighted average sound level of 85 dBA.  A.	Quest Technology Edge 5 dosimeter (Serial # EST080227) placed on an employee conducting shift lead operations was exposed to noise at a level of 81.1% of the allowable limit during a 480 minute sampling period on March 25, 2024; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 88.4 dBA for the dose and time of the employee exposure.  B.	Quest Technology Edge 5 dosimeter (Serial # EST080228) placed on an employee conducting machine press (steak press) operations was exposed to noise at a level of 160.1% of the allowable limit during a 480 minute sampling period on March 25, 2024; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 93.3 dBA for the dose and time of the employee exposure.  C.	Quest Technology Edge 5 dosimeter (Serial # EST080229) placed on an employee conducting welding operations was exposed to noise at a level of 76.4% of the allowable limit during a 480 minute sampling period on March 25, 2024; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 88 dBA for the dose and time of the employee exposure.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
