29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a)  At 7948 Calverton Street, Lot 13 Primland, Foley, Alabama:  On November 29, 2023; the employer allowed his employee to operate a pneumatic nail gun without eye protection while performing roofing activities exposing his employee to struck-by hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 7948 Calverton Street, Lot 13 Primland, Foley, Alabama:  On November 29, 2023; the employer allowed his employees to perform roofing activities without fall protection exposing his employee to a fall hazard of 9 feet 7 inches to the ground below.

29 CFR  1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support:  a)  At 7948 Calverton Street, Lot 13 Primland, Foley, Alabama:  On November 29, 2023; the employer allowed his employee to use an extension ladder to gain access to the roof and the rails of the ladder did not extend three feet above the roof landing to provide safe access exposing his employees to a fall hazard of 9 feet 2 inches to the ground below.
