On or about October 26, 2023, during a construction project of a commercial building located at 11351 Pearl Rd, Strongsville, OH 44136, the employer did not ensure employees working from a roof were protected from a fall of approximately 12.1 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
