(a) Location - Jobsite  On or about 11/02/2023, an employee was working from the elevated basket of a JLG Model 860SJ (SN 0300217045) approximately twenty-five (25) above the ground level and was observed not wearing a body belt and/or lanyard for personal fall arrest.
