(a) At the Shipyard: On or about 12/1/2023, workers in the yard area were exposed to being struck-by operating forklifts due to the fact that they were allowed to walk and work in the area where the Marine Travelift Forklift, Model #M2800HS, Serial #1213-1218, was operating and the backup alarm was not audible above the sound of the forklift engine by pedestrian employees in the area.
