OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards. 1) Warehouse/Dock Area: On or about December 7, 2023, an employee was observed operating a forklift (#29 Doosan D35 /SN: #35NC7 0807) without wearing a restraint system (seatbelt). 2) Dock Area: On or about December 7, 2023, employees who operated the two Caterpillar forklifts (GP35N/SN: # AT13G50015 & SN: #AT13G50247) did not use the restraint system (seatbelt). It was secured/interlocked behind the operator's seat.
