Patterson Rig 289: On or about November 28, 2023, the employer did not ensure that employees were protected from tripping into or stepping into a hole located in the Mud Pits (Tank #6). This condition exposed employees to tripping hazards.

29 CFR 1910.335(b)(1): Safety signs and tags. Safety signs, safety symbols, or accident prevention tags shall be used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145. Patterson Rig 289: On or about November 28, 2023, the employer did not ensure that an electrical panel located at Crew House #1 had signs to warn employees about electrical hazards. This condition exposed employees to electrical shock hazards.
