Employees exposed to burn/molten metal splash hazard as they were observed wearing worn, ill fitting, or incomplete sets of Foundry Green, Flame Retardant clothing while furnace tending and during molten metal pouring operations.

All deficiencies such as listed were not being carefully examined and determination made as to whether they constitute a safety hazard:  1910.179(j)(2)(i) All functional operating mechanisms for maladjustment interfering with proper operation. Daily. 1910.179(j)(2)(vi) All functional operating mechanisms for excessive wear of components  On or about December 04, 2023, the employer did not ensure the overhead crane used to transport molten metal in a ladle to casting stations was inspected for defects, exposing employees to possible burns from molten metal. Deficiencies were not carefully examined and determination made as to whether they constitute a safety hazard in that the crane was being operated with a faulty rail system.

29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed:  On or about December 04, 2023, unsafe conditions were disclosed during the frequent inspections by the employer of the overhead crane utilized to transport molten metal in a ladle to casting stations.  A deteriorating support wall was causing the tracks of the crane to shift, leading the crane to seize when traveling down the tracks exposing employees to molten metal burn hazards. The employer did not correct the deteriorating support wall before the operation of the overhead crane resumed.
